The TCD form reads “PERMIT NO: 1915 Date Issued: 9/9/2025” and is issued “To: Sherwood” (`SP-06-2025-0042_City_of_Sherwood.pdf`, p. 10). District special permit SP-06-2025-0042 is dated October 6, 2025 (same file, pp. 1, 5). The site corresponds to Sherwood's current inventory site 15, Highway 107 at the bridge south of General Samuels. It must not be merged with case-261931 historical #07 at log mile 11.419 (`34.89850692, -92.17196590`). ## Permittee, contractor, and bond The local authority is the permittee: ARDOT's terms state, “Permits shall be requested by and issued to the local City Mayor or County Judge” (`1105497_-_Signed_docs.pdf`, p. 2). Flock is the contractor/installation and bond party, not the municipal permittee; the special permit says a $1,000 bond “has been received from Flock Safety” (`SP-06-2025-0042_City_of_Sherwood.pdf`, p. 4). ## Change order and completion CO1 was approved as noted on January 13, 2026.
